Mutli-Billion Dollar settlements (as partial payment for the variety of [preventable] injuries of all the innocent victims – that nearly bankrupt the Corporations and their Insurance carriers – is about the only thing that will fix the "bottom line" fixated decision makers.

Only companies that run by workers will fix this. Nothing will change until the workers, not profits, will be able to hold and control all aspects of life themselves.
Listening to the retired union representative it is apparent neither railway board members, nor judges presiding over court hearings understood / understand how railways work. When it is explained the trains are too long and too heavy for the system to operate safely that is a technical fact too difficult to understand. Surely not, a child could understand. There has to be some other reason for not understanding. Perhaps it's wrong headed intent, profiteering regardless of safety concerns, government complicity for bribes – votes, the system? The latter seem to fit the facts. It can be summarised as evil at work and should be treated and prosecuted as such. I don't know if that's possible in the USA of today?
